These guidelines are designed to facilitate the completion of the Heart and Stroke Foundation of Ontario Program Grant Full Application Form. Applicants are advised to read both the following and the Program Grant Terms of Reference carefully. A copy of the component Grant-in-aid (GIA), and where applicable, any progress reports must be appended to form part of the Program Grant application.
There should be no duplication between equipment or other research funding requests for the component GIAs and for the Program Grant.
The complete Program Grant Application should be received by the Heart and Stroke Foundation of Ontario, Research Department no later than November 30, 2007. Late applications will not be considered.
N.B. Program Grant Letters of Intent are to be received by the Heart and Stroke Foundation of Ontario, Research Department no later than June 29, 2007 for renewal applications and July 13, 2007 for new applications.
Number of copies and Mailing Addresses
One original collated copy of the Program Grant Full Application Form, must be forwarded to the Heart and Stroke Foundation of Ontario, Research Department. Seven copies of the full proposal must be forwarded to the Heart and Stroke Foundation of Canada.

The Director (Principal Investigator) of the proposed Program Grant should complete this application form.
The applicants are recognized by the Heart and Stroke Foundation of Ontario (HSFO) as the authors of the intellectual content of the application submitted. The HSFO will correspond with the Director of the Program Grant who is responsible for communicating all Foundation decisions to the Program Grant team members.
The institution where the Director of the Program Grant holds an academic appointment must acknowledge that the Director is responsible for directing the Program Grant, including the coordination of all core activities. The institution is also responsible for the provision of adequate space, facilities, furniture and general services as required by the Program Grant Director.
The Financial Officer of the institution where the Director of the Program Grant is located is responsible for administering the Program Grant budget, in addition to the Director’s component GIA.
Each individual component GIA will be administered by the institution where the Principal Investigator of the component GIA holds an academic appointment. Each institution continues to be responsible for the provision of adequate space, facilities, furniture and general services for the component GIA and the Program Grant.
Program Grant composition
List all the component GIAs proposed for inclusion within the Program Grant. For each GIA, provide the name of the principal investigator, names of co-investigators, project title of the GIA, start and end dates of the grant, the grant types (new, continuing or renewal), and the funding agency. Add pages as necessary.
Append a copy of the component Grant-in-aid (GIA), and where applicable, any progress reports that form part of the Program Grant application.
Lay Summary of the Program Grant Proposal
This should be written in lay language in a style suitable for a press release.
Objectives of the Program Grant
Please provide a concise outline of the objectives, hypotheses, approaches and research plan of the proposed Program. Give a brief description of the current state of knowledge and how it is anticipated to further develop the understanding of the subject. Also highlight how each component GIA contributes to the overall objectives of the Program Grant. (Maximum 5 pages)
Progress Report For renewal applications, please list original objectives and achievements accomplished to date against these objectives. (Maximum 1 page)
Please provide a communication plan describing how the individual investigators will interact to form a cohesive team of individuals working on the objectives of the Program Grant.
Discuss the core facilities as they relate to support for the objectives of the Group. Provide details to demonstrate the Institutional direct commitment to the Group. Letters from the appropriate authorities should be attached to the application.
Discuss the training environment within the Program Grant and the projects trainees will be working on.
Response to Previous Reviews
Applicants may respond here to reviews on previous program grant applications. ONE (1) additional page may be added.
Funds can be used to support items which are of direct benefit to the Program Grant such as: infrastructure costs, salaries of research trainees, assistants and technologists; equipment and maintenance contracts; materials and supplies; investigator travel; and other shared expenses.
Direct costs of each component GIA, as well as salary support for investigators are not funded by the Program Grant.
Full justification of all items requested must be provided. In the event that personnel support is requested, a copy of the individual’s curriculum vitae should be attached to this page.
For equipment items included in the budget, please indicate:
- the availability and status of similar equipment
- the reasons for choice of specific type, model or service contract, in relation to alternatives (provide current and detailed price quotation)
- where applicable, the necessity of upgrading existing equipment or service contract
Other Funding Sources
List other Program Grants, component GIAs, and/or Centres of Excellence currently held or applied for from this and other grant agencies for research by the Director of the Program Grant and/or component GIA Principal Investigators during the term of this award, if approved. A short summary and approved budget must be provided.
Milestones and Outcomes
List the overall vision for the program, milestones, and key outcome indicators. Indicate the value added for this Program Grant.
The signatures on the application commit both the applicants and the institutions to specific responsibilities. The applicants and the institutions are jointly responsible for adherence to the general regulations governing the award of a research grant as outlined in the Heart and Stroke Foundation of Canada Guidelines for applicants for the 2008-2009 Funding year.
Original signatures are required from the Vice President Research or authorized delegate representing the institution. If the applicant is normally the signing authority for the department of the faculty, another senior official must sign instead. The signature certifies that:
- the budget estimates are in accordance with the institution’s rates and policies
- the institution is willing to administer any grant received according to the Foundation policies as set out in the Heart and Stroke Foundation of Ontario Regulations Governing Research Grant Awards, the Heart and Stroke Foundation of Canada Guidelines for Applicants, and the HSF Research Integrity Policy
- the institution will release funds to the applicant only when all conditions have been met, and will invoice the HSFO in arrears for the approved expenses
- the institution will notify the HSFO of any change in the status of the Program and/or applicants during the tenure of the grant
Original signatures are also required from the Director of the Program Grant and the Principal Investigators of the component GIAs. The signatures confirm:
- the accuracy of all information provided
- acceptance of the terms and conditions of the Program Grant
- that the applicants have read and agreed to comply with Foundation policies as set out in the Heart and Stroke Foundation of Ontario Regulations Governing Research Grant Awards, the Heart and Stroke Foundation of Canada Guidelines for Applicants and the HSF Research Integrity Policy.
Payment administration of grant funds
Designate the institution to receive and administer the grant funds.
