PDF - Portable Document Format

PDF is the abbreviation for Portable Document Format. A portable document format is a type of file that allows you view/print documents on any platform (Windows or Macintosh). PDF documents appear identical to the original documents because they maintain the exact formats, fonts, images and colour of the original documents. Please note that to view a PDF file you will need a copy of the Adobe Acrobat Reader software. This software can be downloaded from the Adobe Systems Incorporated website at no charge.

The Heart and Stroke Foundation of Canada has made its guidelines and application forms available in PDF. In doing so, applicants can now download a copy of our guidelines and/or forms which they would then print. The printed forms will be identical to the hard copies that would normally be available through the Heart and Stroke Foundations or your institution and can be used for submission of applications.
