Installation of the Heart and Stroke Foundation of Canada Application Forms in MS Word 97 for Windows

  1. Download the appropriate file (i.e. "GIA11.doc", "SP11.doc","JP11.doc","RS11.doc" or MS11.doc).
  2. Start the MS Word program, and "Open"  or double-click on the desired form.

    To open the Grant-in-Aid form select "GIA11.doc"
    To open the Senior Personnel form select "SP11.doc"
    To open the Junior Personnel form select "JP11.doc"
    To open the Research Scholarship form select "RS11.doc"
    To open the Master Studentship form select "MS11.doc"

Additional Instructions for using MS Word and the HSFC Application Forms

Format limitations

The HSFC forms created in Word 97 will not allow formatted text (i.e. does not allow bolding, italicizing, underlining, etc). Any text typed or pasted into the forms will become plain text. Therefore, you may wish to complete and print the "Detailed Proposal" and/or "Progress Report" using a separate word processing document. You will then need to collate these additional pages with the Word application form after the form has been printed. You may use word processing software other than Word (ie. WordPerfect) to complete these additional pages. However, you must:

  • use 21.25 X 27.5 cm (8 1/2" by 11") paper
  • use a margin of 2 cm (3/4 inch)
  • use either Times New Roman 12 point or Arial 11 point font,
    condensed spacing is not acceptable.
  • print on one side

Please note that the font on the application form in the Detailed Proposal and Progress Report sections is pre-set at Courier 11 point font. It will be acceptable if you choose to use the page provided on the form (with its Courier 11 point font). Note however, that all additional pages must use either Times New Roman 12 point or Arial 11 point font. Alternatively, you may type "Please see attached" on the "Detailed Proposal" and/or "Progress Report" section and attach the additional pages using the format specified above.

For the GIA program, please follwo the instructions provided in the application form and Guidlines for the "Detailed Proposal" section.

 

 

Research Classification List
When referring to the research classification list to select the appropriate entries for level of organization, methodology, specialization and disease, please enter the abbreviated codes rather than entering the complete words.

For example:
Levels of organization - enter "3.1a " to represent "Central Nervous System molecules/cell/tissues"

Methodology - enter "M-20" to represent "Echocardiography"

Specialization - enter "S-40" to represent "Neurosurgery"

Disease - enter "D-5" to represent "Atherosclerosis"
